By providing my phone number, I agree to receive automatically-dialed phone calls, pre-recorded phone calls and text messages, including, without limitation for example, SMS and MMS messages, from or on behalf of Everly, LLC and Security Benefit Life Insurance Company (“SBLIC”) at the number(s) I provide. I am the subscriber and sole user of the telephone numbers provided. Everly and SBLIC may place such calls, including text messages to the number I have provided to assist me with my application, verify my identity, service my account, or provide me with marketing offers or promotions regarding their products or services and education/ideas that might interest me.

I agree that if I do not wish to continue receiving further calls or text messages from Everly and SBLIC I will notify them in writing at privacy.inquiry@everlylife.com. I understand that my consent is not a condition of this transaction.

I also agree that by providing my email address, I am agreeing to receive marketing offers or promotions regarding Everly and SBLIC’s products or services at my email address.
