Notice and Consent to Doing Business Electronically

By entering your name and other information, you are electronically signing the authorizations we seek and, if applicable, your application. In addition, you are affirmatively and voluntarily consenting to the use of your

electronic signature, the electronic submission of your application and related forms, and our electronic delivery of all documents, which Everly, LLC (Everly) or Security Benefit Life Insurance Company (SBLIC), as applies, will provide you related to the policy. Our electronic delivery of documents may include cancellation, nonrenewal, and privacy notices, unless otherwise required by relevant laws and regulations.

As used in this “Notice and Consent to Doing Business Electronically,” “we” or “us” refers to Everly, Everly’s affiliates and their agents, SBLIC, and SBLIC’s affiliates and their agents.

You understand:

  • your electronic signature will be legally binding and enforceable as if you had signed a paper authorization, application, and related forms, as applicable.
  • if SBLIC accepts your application, an electronic copy of the completed application and other forms bearing your electronic signature will be provided at the time of the electronic delivery of the policy.

If SBLIC does not accept your application, an electronic copy of the completed application and other forms bearing your electronic signature will not be provided, unless you request by contacting SBLIC at: Security Benefit Life Insurance Company, PO Box 758605, Topeka, KS 66675-8605 Attention: Underwriting/New Business Department;

You may elect to be provided a free paper copy of the completed application and other forms bearing your electronic signature, and all other related documents by contacting SBLIC at: Security Benefit Life Insurance Company, PO Box 758605, Topeka KS 66675-8605 Attention: Underwriting/New Business Department.

Your consent confirms that you have a device with access to the Internet to accept electronic delivery of all documents and the device allows you to read and retain any document saved in a portable document format. The hardware and software required for electronic delivery, access, and retention is set forth below.

You have the right to withdraw this consent to do business electronically with us at any time, which includes receiving documents electronically. If you withdraw your consent, the withdrawal applies to electronic transaction with Everly, Everly’s affiliates and their agents, SBLIC, and SBLIC’s affiliates and their agents. You have the right

to update your electronic contact information. To withdraw this consent or to update your electronic contact information, please contact Everly/SBLIC at (855) 290-0529. If you withdraw your consent to do business electronically, we will provide the documents we deliver in hard copy to your last known address.

If you withdraw your consent, from that time forward:

  • You will be unable to apply for products and services, including insurance products, through Everly,
  • We will be unable to continue to process any pending applications for products and services, including insurance products, and
  • You will be unable to authorize any transactions with respect to your policy electronically, unless you subsequently consent to do business with us electronically.

Hardware and Software Required:

In order to accept electronic delivery of documents and to retain such documents, you must have a device with internet access, and the following, based upon device:

Computer:

  • Browser support of Internet Explorer 11 or higher, Firefox 85 or higher, Safari 14 or higher, Chrome 88 or higher.
  • Sufficient electronic storage capacity on your device or other data storage; and
  • An active e-mail account that can access the internet
  • A US/Canada (+1) mobile phone number that can receive text messages
  • Javascript must be enabled; and
  • Third party cookies must be enabled iOS Device:
  • iOS version support 14 or higher. Android Device:
  • Android version support 11 or higher.

We will update you if there are any changes to the hardware or software requirements that could impact your receiving electronically delivered documents.
